LA City Care HSC offers a comprehensive range of services delivered by community-based, mobile professionals or multidisciplinary teams, including trained support staff. These services are tailored to meet the unique mental health needs of each client. They are available for adults, children, adolescents, and families, with the type, hours, and intensity of services adjusted to reflect the evolving needs of each individual.
The primary goal of Community Psychiatric Support and Treatment (CPST) services is to provide specific, measurable, and individualized support to each person served. These services are designed to enhance an individual’s ability to thrive in their community, access essential resources, and achieve meaningful improvements in areas such as education, employment, family relationships, and community integration.
- Needs Assessment and Independence: Continuous evaluation of needs and assistance in achieving personal independence for managing basic daily requirements, as identified by the individual and/or their parent or guardian.
- Skill Development: Facilitation of the development of daily living skills, if identified as a need by the individual and/or their parent or guardian.
- Individualized Service Plan (ISP) Coordination:
- Management of services outlined in the ISP.
- Support with accessing natural support systems within the community.
- Establishing linkages to formal community services and systems.
- Symptom Monitoring and Crisis Management:
